PREAMBLE

The purpose of this evaluation is to be based on your report related to feedback on management prior to the end of year performance evaluation. to provide an opportunity for the employee and his /her supervisors to engage in discussion regarding progress on expected performance and the work environment. It will give an opportunity for the supervisors to acknowledge and encourage good performance and also address possible performance gaps while jointly curbing the prevailing challenges.
